On occasion, when you double-tap on an icon on the home screen in order to open an app, VoiceOver will stop speaking and become unresponsive for a short time, usually about ten to fifteen seconds.

In the News app that’s new in iOS 9, a 2-finger swipe down (to have VoiceOver ‘read all from current position’) inside a story will sometimes cause VoiceOver to crash and restart.

The Today screen is a page to the left of the first home screen which is dedicated to hosting widgets.
It is possible to have a large number of widgets, in which case you can scroll up and down the page to reach more.

After clearing a notification from the Lock screen, VoiceOver may behave as if it were still present on screen. Specifically, after using the “Clear” option from the VoiceOver rotor, the notification is visually cleared from the Lock screen. However, when moving VoiceOver focus by flicking, you may find that VoiceOver navigation and feedback will behave as if the notification is still on screen.

After connecting a supported USB HID braille display via USB and then locking the screen, braille input and output stops.

After connecting a braille display through USB, there is no listing of the braille display in the braille menu under VoiceOver. However, any changes made to the settings of that display still take effect.

After entering customisation mode on the Lock screen, there are some UI elements which VoiceOver users can only locate by touch, not flicking. Specifically, there is a page picker and buttons for “Customise” and “Add” which are visually present below each preview of existing Lock screens. If you flick to move VoiceOver focus through the screen, these are not located. However, they can be located by touch.

After using “More” from the VoiceOver rotor's Action menu on a notification, VoiceOver users can locate when swiping UI elements which are not present visually on screen and do not behave as expected. Typically, this will be other UI elements which are on the Lock Screen or Notification Center such as notifications or playback controls. When double tapping on one of these ‘phantom’ UI elements, instead of performing the expected behavior, the interactive view of the selected notification will be dismissed.

Upon first downloading the Alex TTS Voice, VoiceOver fails to switch automatically from the voice in use while the Alex download was in progress.

It seems that Apple has recently implemented some server-side updates to its Subscription Management display, which have introduced problems for VoiceOver users. As a result, apps or services listed on the screen are no longer accessible or locatable, rendering management of subscriptions unusable for those who rely on VoiceOver.

This behaviour is present if you access the subscription management screen via the App Store or at Settings > Apple ID > Subscriptions

Voiceover navigation has been getting worse in the Apple News application starting at the iOS 14.1 update.
1. It is no longer possible to navigate sections such as recommended for you, top news or even New issues for News+ using the Voiceover heading rotor navigation option. Headings don't appear to be labeled as such.

If you have 2 or more cards, tickets or passes added to Apple Wallet, if you activate Wallet and attempt to use any of these other than the first one listed, Wallet will crash when you double tap on the desired card, ticket or pass.

For some the crash will be immediate, for others there will only be a crash if you perform a swipe gesture after activating the lower positioned card.

When using Apple AirPods (confirmed with the 2nd generation), the Voiceover rotor contains an audio destination option which does nothing when being interacted with.

Audio levels don’t always return to correct level after being interrupted by VoiceOver when audio ducking is enabled.

You may be listening to music. The audio level is reduced when VoiceOver needs to announce something. But, after the announcement, the audio level stays at the ‘ducked’ level rather than reverting back to the previous level.

On occasions, audio will begin to break up and "crackle (this has been seen with the device's internal speaker, and when playing audio through a connected speaker or headset)." We have found that toggling VoiceOver off and back on again will typically resolve this issue.

When using an app and a banner notification is displayed, you may find that on occasions the banner will remain on screen until you either interact with it or switch to another app or the Home screen.

Note that this is when you have the banner style set to be ‘temporary’, which means that it should only be displayed on screen for 2-3 seconds before automatically being hidden.

This appears to be a VoiceOver-specific issue, as we have not seen reports of sighted users encountering this behaviour.

Things appear to work as expected if, after triggering the 3D Touch menu, you keep your finger pressed to the screen and drag to locate a menu item by touch or perform a very distinct swipe gesture to move VoiceOver focus. However, anything else (such as lifting your finger and then trying to locate a menu item by touch) may result in one of the menu items being triggered with no input from you.
