Good morning,
Recently, after logging into my Mac Mini, I keep getting this window that appears, which VoiceOver reads as "Accessibility UI server has no windows". Since this has only appeared recently in the past week, I'm not sure if this is a glitch with Tahoe or some unwanted setting that's enabled/disabled.
Just curious if anyone else has seen something similar and perhaps may have a clue as to what might be going on?

I get the issue on both a Mac mini M2 Pro and a MacBook Air M1 running Tahoe.
It may be a Tahoe bug!
Since it appears I'm not the only one, it sounds like it may be a bug then.

Tahoe does seem to offer different experiences across users of Voiceover. I am not going through the issue of focus jumping in Safari and this issue you are talking about too.
Yes, I’m experiencing exactly the same issue.
The only difference for me is that it tells me the system speech synthesizer has no windows. This happens every single time I start up my Mac. The message appears right after I enter my password. Maybe it’s some kind of bug, and that’s why I can’t speed up the system speech synthesizer in any way. This bug is really frustrating for me because I can’t create subtitles for videos.

I'm not getting this, but I have filevault enabled, could that be only triggered when if one doendoesn't have filevault enabled? Just a guess. Also, did it start on mac os 26 or did it start happenning specifically after 26.1?
I have the firewall enabled, and I’m experiencing this problem.

That’s probably not the cause, though sometimes having the firewall on does create issues. For example, in my case the login screen was read with a German synthesizer, and the same thing happened with system recovery. Fortunately, there is a solution to this problem.
File Vault
The issue occurred some time after updating to Mac OS 26, and I think began after I disabled the File Vault. Not sure why that would make a difference, but if some others are also experiencing something similar, this could be a bug with Mac OS Tahoe.
I was surprised that, even though I turned File Vault off originally, Apple decided to turn it back on, basically overwriting my preferences.
I didn't realized until later that File Vault was re-enabled as, after updating, when arriving at the login screen, instead of hearing my preferred VoiceOver configuration coming out of my external Bose Companion 2 speakers, I was met with a slow, quiet basic Samantha at the login screen, which was coming out of my Mac Mini's built-in speaker.
Once I disabled File Vault though and rebooted, then all returned back to the way it once was.
I understand that for some, File Vault is a must, and that's alright. I just know that it's not for me. Back when I got my first Mac, a 2017 MacBook Pro, I listened to David Woodbridge's podcast on setting one up and his recommendation was to keep this feature disabled, so I just kept it that way.
As for the Accessibility UI Server window, I get that it might be a bug, since some others are experiencing a similar situation.
I have file vault disabled.
I'm not getting anything like this, I don't think. But I've only restarted the Mac once, I tend to just put it to sleep nowadays and restart if something's weird, just in case a restart might help.
