Hi! I am using a bluetooth keyboard for my Ipad. I have two keyboards and the same thing happens on both. Often when I press VO+Space I often get a double tap and hold instead of a double tap. Is there a setting somewhere where I can fix this because it is messing up a lot for me. Both my keyboards are different brands so I don’t think it is the keyboard.

It is the focus issue I think!
I have been facing similar issues with my keyboard. I have IPAD pro M4 and Apple’s Magic Keyboard and a trashy Bluetooth keyboard of my office. I think VO jumps focus while typing on the keyboard with IPad. This has been an issue since the first IPad Pro 9 inches which I got in 2017. Despite repeated feedback to apple and the accessibility team about it, nothing has been done about it till date! I am hoping the IPad OS 19 (which is supposedly will give us desk mode) will fix it. Fingers crossed! The workaround I found was to pair and re-pair the keyboard. I’d appreciate it if more users comment here and tell us whether they’ve been facing similar issues and possible work-around for it.
