Do you Sometimes Use a Bluetooth Keyboard? If you sometimes use a bluetooth keyboard then this will make the onscreen keyboard disappear. Pressing the key on the upper right of my Apple Bluetooth keyboard has voiceover on my iPhone 4s cycle between "Keyboard visible" and "keyboard hidden."
