On the remote, the bottom left button can turn on Voice Over. Press and hold it for a few seconds, and a menu comes up. If you are at the main menu screen, the first option will be Voice Over. Press the top center select button to turn it on.

One caveat. Every once in a while, I find the long hold of the menu button to be ineffective. The only solution I've found is to disconnect the Apple TV power cable, and then reconnect it and let the box reboot. The, try the long press again.

Keep in mind the accessibility menu needs to be enabled.

It's in settings, general, towards the bottom.

Make sure it's on or you'll either have to rely on sighted help or memorise the menus just to turn voiceover on.
