I had this issue earlier and when searching on the applevis website, I found my own solution but this time it failed to help me out.
I have my text messages to notify me on the locked screen. But every time I get a text message, the locked screen wakes up and Voiceover is happily reading my text message loud enough to be heard in some remote town in India, I mean even the cows have heard what the balance in my bank account is.
I have checked under Settings/ accessibility/ Voiceover/ Verbosity and under System notifications it is set to do nothing...
Yet it speaks - HELP!!!

The easy way, and the hard way...
I am fairly certain you have already done the easy way, but I will suggest it hear regardless.
The 'Easy' way:
Go to Settings, Notifications, and turn the 'Allow Preview' setting to 'Never'.
The 'Hard' way:
Go into every, single, item that you are allowing notifications for, and under the Alerts heading for each one, uncheck 'Allow on Lock Screen' and 'Allow Banner'. Only leaving 'Allow in Notification Center' active.
The second option is a pain in the backside, but will guarantee that you will not get any notifications on your lock screen, or any banners on your home screen. Of course, if you do want them on the lock screen, then leave that option alone, and vice versa for the banners.

Thank you for suggesting this but does it not really miss the point of having the ringer mode VS silent mode? If this is a bug, are you ware of it being reported to apple either through applevis or any other forum??

What Worked for Me

I have seen this exact issue, and I believe in its current form, it is related to the 'Braille' option being on for speaking of system notifications when the screen is locked. In my experience, it will reliably present after (1) resetting VoiceOver settings to default, or (2) setting up a device as new without restoring from backup.
Here's what worked for me. Note that if you have a braille display, I believe this will disable the presentation of system notifications in braille when the device is locked.

Dear Michael, thank you for your comment I have done what you suggested and am now observing the behavior of VoiceOver with notifications on the locked screen. I asked a friend of mine to send me a text message and Voiceover did not read the SMS/ Text message on the locked screen, but I did get my Text alert tone, which is really neat. I also made one more setting that might have helped, not sure where exactly in settings but it has to do with locked screen preview the options were always/ when unlocked and Never. Sorry it's really late in the night as I reply to this comment and I have lost memory of where the setting is.
