Problems with Braille displays and accessNote

Hello all,
Recently my wife and I have experienced a problem when using our Braille displays and accessNote. Here is what happens:
1. Add a new note and begin to write.
2. At some point while writing, the Braille display will revert to a previous line, making it impossible for you to see what you have just written.
3. Use dots 3-6 and space bar to move the insertion point back to the end of your current writing position in order to continue.

Has anyone else experienced this? Both iphones (my wife's and mine) are using the current versions of iOS and the app.
