If you have an alarm set to go off on the Apple Watch, a few seconds before it starts, the iPhone will wake up and speak.
The iPhone should not wake up or make any sound, as the alarm is set to use the watch.

Ensure VoiceOver is enabled on the iPhone.
Go to the Health app and set up a Sleep schedule.
Ensure the alarm is set to go off on your Apple Watch. In my case, I have chosen vibration only, no alarm sound.
Go to sleep wearing the Watch.
In the morning, a few seconds before the alarm starts up on the Watch, the iPhone wakes up, and VoiceOver speaks. This means that instead of being woken by the gentle vibration on my wrist, I am woken by VoiceOver speaking on my phone.
Turn off VoiceOver on the iPhone before going to sleep.
