Original Comments: For the most part this app is very accessible. However, there are a few issues for VoiceOVer users.
The first issue is that VoiceOver does not read timestamps in the timeline. This might seem a minor problem, but I suspect that it will be enough for many VoiceOver users to disregard this app. If you double tap on a tweet in your timeline, the tweet opens in its own page, on which the timestamp is read.
Another problem is evident on this tweet page. There seems to be no obvious way of tapping through to the user's profile page. However, although the avatar image is not recognized by VoiceOver as an active page element, double tapping on it will take you to the user's profile.
Unfortunately there is another issue on other users' profile pages. If you tap at the top of the page and swipe right, VoiceOver will recognize the avatar and then the user's display name. VoiceOver will then behave as if it had reached the last item on the page, even though there is still lots more on the page. Tapping further down on the page will move the VoiceOver focus, and you can now swipe through everything on the page. Even if you go back to the top of the page, swiping right will now give you access to everything. From what I have seen so far, this problem only occurs the first time that you access the user's profile page. If you navigate away, and return back later, the page seems to work perfectly with VoiceOver.
If you are not bothered by the timestamp problem, I would certainly suggest that you take a look at the is app, as it has quite a clear and easy to use interface.

#1 Don't update to 4.1
I've just downloaded the update to version 4.1, and this has major accessibility problems.
The timeline is now mostly inaccessible. You cannot swipe into or through the timeline. Tapping anywhere on the timeline will have VoiceOver read the text from the line that you happen to have tapped on, but nothing more. You will not be able to swipe to read anything more.
#2 Version 4.2 looks very good
The latest update to Twitterrific specifically mentions VoiceOver fixes in its release notes, and I have to say that I am really impressed with how the app now works.
It doesn't have all the features of TweetList, but I do find it more intuitive and generally easier to use. The interface is fairly minimal, whilst a double-tap on a tweet brings up a menu of every available action.
I like the way that VoiceOver reads tweets as you flick through your timeline. It tells me everything I want to know about the tweet, in the order I would like it. I particularly like that it states when a tweet is part of a conversation.
Like TweetList, it does not support push notifications, so you would need to use an app such as Boxcar to manage these.
