The newest update seems to be completely accessible, even optimized for VoiceOver.

#1 Those unlabelled buttons
The unlabelled buttons in the list view of stations are used to select which stream you want when the station has more than one.
As VoiceOver gives you no feedback on what the choices are, this is very much a lucky dip.
The developer has been contacted, so hopefully this will be addressed in a future update.
#2 The problem with those
The problem with those unlabeled buttons has been fixed in version 2.2.
#3 Watch out for a certain button
A friend of mine used tune in radio and clicked on a button called speaker-high. When she did, it disabled voice over and she had to totally restore her ipod before it worked again. Even then, voice over sounded funny. So whatever you do, do not doubletap that button!
#4 Some... People!
That "certain "speaker high" button" is a mute button, to get sound back all you have to do is raise the volume. She went through way, way way way too much trouble.
And people, you may use that button, just remember that it lowers down the volume completely.

But yes, that is a mute button. Double tap it, it'll mute. Double tap it again to get the volume back. Or as Kevan said, just use the volume buttons.
#6 Speaker High is really just a paper tiger. Not really dangerous!
Hello everyone. I got scared with the speaker high button some time ago but didn't feel lead to share my experience because for me, the problem was over as quickly as it began. My iDevice is the iPhone 3GS. What I discovered is the speaker high button is very simply a mute button and what they do to accomplish the mute is slam all the volumes to zero. The fix was rather easy for me both when I had the trouble by accident some weeks ago and when I did it on purpose just now to see if I remembered what happened right before posting this. I simply pressed and held the volume up button on my device for a couple of seconds and all was right with the world again.
In my test just now, I did two things. First, I hit speaker high while a stream was playing. It all went silent as I thought it might. I hit volume up and the stream and VoiceOver came back. Second, I stopped the stream and hit speaker high again. Of course, Voiceover stopped as expected. I pressed and held volume up for a short time, did a VoiceOver gesture and it worked just fine.
All of that said, I completely understand how this speaker high button could scare the devil out of anyone. It's not a big leap to at least assume that if you double-tap a mute like button after you mute that it will unmute. This particular button however, does not behave that way so before you restore your device after hitting speaker high, try turning up the volume first.
Finally, I have sent an email to the developer recommending they relabel the speaker high button to mute and explained the trouble mentioned in the previous comment.
